Instructions for I-9 Completion

Employment Eligibility Verification Form (I-9) 

 - Review employee instructions for I-9 completion

 - Complete PAGE 1 of the I-9 form, print pages 1 -3, sign and date PAGE 1 (please print   double-sided if possible)

 - Review Page 4 of the I-9 form for acceptable documents to submit along with the I-9 form   at your new employee orientation

Federal Employee's Withholding Allowance Certificate

 -When completing your Payroll tax forms, you may want to refer to your tax preparer/advisor for guidance on completing the forms. Please be aware that the West Bend School District cannot advise employees on how they should complete their tax withholding forms. 

 - After Completed, PRINT PAGE 1, sign and date. To help you determine your tax   withholding, you can utilize the IRS Tax Estimator.

 - Please complete top portion of the form. 

Direct Deposit

 - Attach a voided check to the completed form
